In an effort to maintain a healthy environment for our employees and guests during a time of heightened awareness, Alexandria Memorial Gardens is requiring our client-families and vendors to provide information regarding any potential exposure to COVID-19, by completing and submitting this prescreening visitor form. Thank you for assisting with our efforts to keep yourself and everyone in our environment safe and healthy.

This pre-screen survey is a precautionary measure to ensure our staff remain healthy and our workplace continues to be a place that provides essential services for our client-families and community. Please kindly provide your travel history and exposure potential below:

Have you or anyone in your household traveled outside of the United States within the last 21 days?
Have you come into close contact (within 6 feet) with someone who has a laboratory confirmed COVID–19 diagnosis in the past 14 days?
Do you have a fever (greater than 100.4 F or 38.0 C) OR symptoms of lower respiratory illness such as cough, shortness of breath, difficulty breathing or sore throat?
Have you experienced a loss of smell and/or taste in the last 14 days?

Act 417 of the 1974 Louisiana Legislature created the Louisiana Cemetery Board. This Act now constitutes Title 8 of the Louisiana Revised Statutes, consisting of Chapters 1 through 13, both inclusive and Sections 1 through 906, both inclusive.
